To determine the degree of underreporting of alcohol as a risk factor for trauma in the emergency service, and to identify factors that might help to explain this underreporting. A case note study was conducted to determine the frequency of alcohol intoxication recorded at reception in north-east London. Analysis of the case notes was undertaken to identify factors associated with evidence of underreporting of alcohol in patients with an International Classification of Diseases-10 diagnosis of injury. Analysis of one month of incoming trauma patients recorded that 4.1% of the cases were alcohol positive. Alcohol analysis showed that 14% of patients had levels of alcohol above 1.2 mg/mL. Alcohol was cited as a factor in the causation of 80% of the trauma cases. Injuries sustained in collisions with other road users were most commonly cited as 'avoidable'. Head injuries were most frequently attributed to falls. Injuries sustained in falls were cited as being avoidable in 82.9% of cases. Alcohol was not cited as a factor in causation by paramedics in the emergency service. The results are consistent with underreporting of alcohol as a risk factor in relation to trauma in south London. Over half of patients who admitted that alcohol was the cause of their injury did not cite this as a factor in the causation of their injury.
